1. Download the form in Rich Text Format (booking-form.rtf).

2. Fill out the form and send it, in one of the following ways:

A. Type your information in the form's gray areas using a word processor. This way, you may either save the completed form and send it as an e-mail attachment to hhijazi@lau.edu.lb, or, you may print out the completed form then fax it or mail it to the address indicated on it.

B. Print the blank form, fill it out in BLOCK LETTERS and fax it or mail it to the address printed on it.

Option A is recommended, for reasons of clarity.
