The American College of Clinical Pharmacy is approved by the American Council on Pharmaceutical Education as a provider of continuing pharmaceutical education. Participants who fill the verification for attendance form and return it to registration desk, or, mail, or fax by June 1, 2004 may receive up to 12.5 contact hours.
